How do I make a referral to Well-Mind Occupational Therapy?

At Well-Mind Occupational Therapy, we make the referral process simple and straightforward. If you or someone you know could benefit from our services, you can refer them easily by contacting Mairead via phone, email or via the 'Contact us' section on the website, for a referral form. 

Referral Criteria

Our referral criteria is  based on the individual's occupational needs. I.e. the extend of difficulty experienced with daily life skills/tasks and the impact on phyiscal and/or emotional wellbeing.

 

Well-Mind Occupational Therapy can help if you are experiencing frequent difficulties with; 

  • Self-care 
  • Leisure 
  • Motivation 
  • Low mood 
  • Self-confidence 
  • Self-esteem 
  • Balance and coordination - clumsiness 
  • Handwriting 
  • Planning and organising 
  • Attention
  • Sensory processing and modulation 
  • Meal preparation 

 

Unfortunately Well-Mind Occupational Therapy is not the best service to meet your needs if the person seeking referral;

  • Is currently feeling suicidal or participating in self-harming behaviours.
  • Is currently attending Mental Health Services, or is within 6 months since discharge from mental health services. 

A referral at a later date is always welcomed.

Anyone concerned about their or someone's ability to perform daily activities can initiate a referral to Well-Mind Occupational Therapy.

 

Referrals are welcomed from various sources such as;  self-referrals, family members, healthcare professionals, schools, or employers.
